Fees and Insurance

Initial Assessment:  $300.00
Individual Sessions: 
           30 minutes: $100.00
           50 minutes: $180.00
           60 minutes: $200.00
Family Session:       $200.00
Couples Session:     $200.00
Stand Alone Assessment and Letter Writing for HRT and Gender Affirming Surgeries (including updates): $300.00
Licensure Supervision: $80.00 per hour
 

     I offer a sliding scale on a case by case basis.  I am happy to discuss this over the phone or         in person.  

     I am a preferred provider for the following insurance panels:

    *First Choice Health Network

    *Providence Behavioral Health
    *United Healthcare Services

For those panels I am not currently on, I am often able to bill as an out-of-network provider. 
    

Appointment Schedule and Policy

Hours/Days:

I will do my best to accommodate clients schedule and need for appointment.  My regular office days are Monday/Wednesdays from 9-4 and Tuesday/Thursday from 9-8.  I may be able to provide other appointment times on a case by case basis.  

Cancellation:

Please notify me 24 hours in advance for cancelled appointments.  If you do not notify me 24 hours ahead of a scheduled appointment, it may result in being charged for a full session.  This fee will not be charged to your insurance.
